General Description

Under general supervision this non-exempt position provides structured mentoring and individualized academic social and behavioral support to youth within a multi-tiered system of support. This is a grant funded position and continued employment is contingent upon funding. Reports to the Director of Community Connections.

Essential Functions

Deliver individualized coaching problem-solving assistance and skill-building interventions tailored to meet the unique needs of each youth fostering personal development and positive behavior change.

Systematically track and document youth school attendance academic performance behavioral progress and participation in community activities; use data to inform interventions; and ensure active engagement with both school and community.

Actively support and encourage youth participation in extracurricular and community events programs and activities that promote social development leadership and positive peer interaction.

Communicate with program supervisors school personnel and community partners to ensure alignment of efforts share updates on youth progress; and collaborate on strategies for supporting youth success.

Perform in-home and school visits to engage with youth and their families building rapport and trust; use visits to provide direct support address concerns and strengthen relationships between youth their families and the community.

Ensure the confidentiality of all youth information managing sensitive information with the utmost care and in accordance with privacy and legal guidelines associated with youth.

Assist with the monitoring and coordination of additional services offered within the Community Connections Program ensuring an integrated approach to supporting youth and families.

Perform other duties as assigned.

Knowledge Skills and Abilities

Required Knowledge

Youth Development Principles Knowledge of the stages of youth development including physical emotional social and cognitive growth. Apply this knowledge to tailor interventions that support youth at various stages of development fostering positive growth and behavior changes.

Community Resources and Services Knowledge and awareness of local community programs services and organizations that can provide youth and families with additional support such as mental health services recreational activities and after-school programs.

Confidentiality and Legal Compliance Knowledge of laws and regulations regarding confidentiality especially those related to juveniles and minors (e.g. FERPA HIPAA).

Required Skills

Judgment/Decision Making Ability to use logic and reasoning to understand analyze and evaluate situations and exercise good judgment to make appropriate decisions.

Communication Excellent ability to communicate listen to and understand directions information and ideas presented verbally and in writing which may include the preparation of detailed reports including numerical information and statistics. Ability to manage a variety of issues with tact and diplomacy and in a confidential manner.

Time Management - Plans and organizing daily work routine. Establishes priorities for the completion of work in accordance with sound time-management methodology and estimated schedules.

Required Abilities

Build Relationships & Trust - Ability to establish rapport with youth and families from diverse backgrounds building relationships based on trust and open communication.

Adaptability - Ability to remain flexible and adaptable in dynamic situations. Youth may experience setbacks or changes in behavior and the ability to adjust interventions and approaches to meet evolving needs is crucial.
