Wage Range:
PT & OT$60 / hour
SLP$55 / hour

Therapist Responsibilities:
- Assess & Evaluate Patients: Conduct comprehensive evaluations to identify physical cognitive communication and/or swallowing needs based on discipline-specific scope of practice.
- Develop & Implement Treatment Plans: Create and carry out individualized treatment plans and rehabilitation programs to address functional deficits and promote independence.
- Provide Therapeutic Interventions: Deliver skilled hands-on therapy and therapeutic activities including exercises manual techniques adaptive strategies assistive device training and communication or swallowing interventions as appropriate.
- Educate & Support Patients Families & Staff: Provide education such as but not limited to pain management fall prevention home safety coping strategies communication techniques and strategies to improve daily functioning and quality of life.
- Monitor Progress & Document Care: Track patient outcomes document evaluations and progress and modify treatment plans based on patient response and clinical judgment.
- Collaborate with Interdisciplinary Teams: Work closely with physicians nurses therapists educators and other healthcare professionals to ensure coordinated comprehensive patient care.
- Must be a graduate from an Accredited Therapy Program and have a Bachelors Degree
- Must possess current licensure for MN ND or IA
- Must possess strong knowledge regarding state federal and local regulations as they pertain to Home Health care.
- Must be capable of performing the essential functions of the job with or without reasonable accommodations.
- Must be able to communicate in English both verbally and in writing and possess sufficient communication skills to perform the tasks required.
