Position Overview

We are seeking a Physical Therapist (or qualified Physical Therapist Assistant where applicable) to join a collaborative and patient-focused healthcare team in a rural medical setting. This role is ideal for clinicians who are passionate about rehabilitation value close-knit teamwork and are open to relocating to a rural community.

The Physical Therapist will provide comprehensive therapy services to help patients improve mobility function and overall quality of life through individualized treatment plans.

Responsibilities

Perform comprehensive patient evaluations and ongoing assessments
Observe patient response to treatment and recommend modifications as needed
Collaborate with physicians and interdisciplinary care teams regarding treatment plans
Provide therapy services for patients with:
- Neuromuscular conditions
- Musculoskeletal disorders
- Integumentary issues
- Pulmonary and cardiovascular impairments
Select and implement appropriate therapeutic interventions to maintain restore and improve patient function
Document evaluations treatment plans progress notes and discharge summaries accurately
Ensure compliance with clinical standards policies and safety regulations

Required Qualifications

Active Physical Therapy license in the state of Colorado
Basic Life Support (BLS) certification American Heart Association
Willingness to relocate to a rural community
Strong clinical judgment and patient-centered care approach
