Position Overview:
Under the direct supervision of the Outreach Cohort Supervisor the Community Health Worker is responsible for supporting Harris County Public Healths outreach activities and serves as the key liaison to educate promote and connect community members to all HCPH programs/services specifically on behalf of the Accessing Coordinated Care and Empowering Self Sufficiency (ACCESS) and Community Health and Violence Prevention Services Programs. This position will primarily work in the community through various outreach activities within Harris County while also completing in-office tasks/projects to support overall outreach team objectives.
Duties and Responsibilities:
Responsible for diverse outreach education and community coordination activities such as canvassing local businesses/organizations attending community events conducting outreach phone calls and giving presentations.
- Serves as a subject-matter expert to meet outreach objectives for HCPH programs and recommends changes to improve outreach strategies and support the creation/updating of HCPH program trainings for the entire outreach team.
- Maintains knowledge/skills to educate promote and connect community members to all HCPH programs/services.
- Tracks collects and submits outreach service data on a daily and weekly basis.
- Builds and maintain relationships with stakeholders and organizations to create and sustain innovative partnerships.
- Create and deliver educational presentations/workshops on public health topics for diverse target audiences and other duties as assigned including special tasks involved with responding to emergency events.

Education:
High School diploma.
License:
Valid TX Drivers License.
Knowledge Skills and Abilities:
Intermediate proficiency in Microsoft Office.
Must communicate clearly and effectively both orally and in writing.

- Ability to maintain confidentiality when dealing with sensitive information.

Preferences
Experience:
Experience withviolence prevention and/or resource referral programs
Certifications:
Community Health Worker Certification (CHW).
Languages:
Bilingual in Spanish or Vietnamese.
General Information
- Position Type and Typical Hours of Work:
This is a full-time regular position.
- Days and hours of work are typically Monday through Friday from 8:00 am to 5:00 pm.
- Other days including weekends and evening hours as necessary for Outreach needs and to attend community events.
- This position requires the ability to travel within Harris County jurisdiction.
This job operates in both field and office environment. This role routinely uses standard office equipment such as computers phones photocopiers filing cabinets and fax machines.
- This is a largely sedentary role; however some duties would require the ability to lift up to 20 pounds and bending & standing for periods at a time.
