Were seeking a compassionate and resourceful Housing Navigation & Case Management Specialist to help individuals and families experiencing housing this role youll provide personalized case management crisis prevention and housing navigation support while building strong community partnerships to expand housing opportunities.
Location: Chicago IL
Salary: $50000-$55000
Key Responsibilities:
- Conduct client intakes needs assessments and individualized care plans
- Provide referrals and linkages to housing healthcare and supportive services
- Deliver crisis prevention and intervention for unhoused and at-risk clients
- Advocate for clients with landlords housing providers and agencies
- Facilitate financial literacy budgeting and credit management workshops
- Build and maintain strong partnerships with landlords and community organizations
- Document services track progress and ensure high-quality case management
Qualifications and skills:
- Associates degree in Social Services (Bachelors preferred)
- 2 years Human Services or Case Management experience (required)
- Knowledge of HIV care and supportive services (preferred)
- Cultural competence with African American Latino and LGBTQ communities
- Strong organizational time management and facilitation skills
- Computer literacy and reliable transportation with valid drivers license
