As a Physical Therapist in our clients Home Health team you will be responsible for providing Physical Therapy services to patients in their homes assessing patient functioning levels and developing treatment programs to improve independence and physical/cognitive functioning. You will also be involved in patient and caregiver education supervision and reporting on patient progress.
Key Responsibilities
Perform Physical Therapy assessments and develop treatment programs
Record and report patients reaction to treatment and condition changes
Instruct patients and caregivers in therapy programs and device use
Supervise physical therapy assistant and when appropriate the certified nursing assistant
Prepare reports and participate in staff and Agency development activities
Schedule and Shift Details
Schedule: Monday-Friday; day shift (start & end times vary based on the patient needs) 1 weekend every 6 weeks (Saturday & Sunday day shift w/scheduled patients) & 2 holidays per year (1 summer 1 winter on a rotating basis); no on-call

Local travel to residential homes
