What Youll Do

As a Home Health RN youll manage a dedicated caseload and provide exceptional care by:

• Conducting start-of-care routine and follow-up visits

• Completing thorough patient assessments in the home

• Creating implementing and updating individualized care plans

• Monitoring patient status and responding to changes in condition

• Collaborating with physicians supervisors and interdisciplinary teams

• Documenting visits accurately and timely in the EMR

• Following all state federal and agency standards of care

• Protecting patient privacy and maintaining HIPAA compliance

What Were Looking For

• Current unrestricted RN license

• Minimum of 1 year of clinical nursing experience

• Associate or Bachelors Degree in Nursing preferred

• Prior Home Health or Hospice experience preferred

• HomeCare HomeBase experience a plus

• Reliable transportation valid drivers license and auto insurance

• Willingness to travel within the service area (approximately 50%)
