AHRC NYC is seeking Direct Care Workers (DCW) to support individuals with developmental disabilities under the age of 21 in the New York State Forster system.
Pay Rate: $20 per hour
SCOPE OF POSITION:
The Direct Care Worker (DCW) will provide support services to individuals with developmental disabilities under the age of 21 in New York States foster care system. Training and support services are provided in the home and/or community and varies based on the individuals needs. The (DCW)works with his/her supervisor on developing an effective service plan and ensures compliance with HCBS Waiver regulations as well as agency philosophy and policies.
ESSENTIAL RESPONSIBILITIES
Supports individuals with disabilities to lead self-directed life and contribute to their communities.
Supports behavior that enhances individuals inclusion in their communities.
Provides direct training and support in individuals home and/or community.
Empowers individuals and their families by providing support and information necessary to make informed choices and decisions.
Helps build individuals selfesteem and develop assertiveness necessary to lead self-determining life.
Uses formal and informal methods to learn about the needs interests and desires of individuals.
Assists individuals in identifying formal and informal supports available in community and from family members and social networks. Works with individuals to develop and sustain relationships with friends family and community members.
Collaborates in professional manner with individuals and others to plan and implement individualized supports that build upon individual strengths choices and capabilities.
Assists individuals to manage every day aspects of life by teaching and providing supports in manner that builds on individual strengths and capabilities and maximizes choices.
Pursues opportunities for individuals professional growth and shares knowledge and resources with others.
Facilitates effective communication with employees and outside agencies for ongoing development of community opportunities.
Participates in special case conferences and staff and parent meetings as appropriate.
Understands challenges individuals face in leading self-directed lives and assists them to overcome these challenges through effective advocacy and facilitation of self-advocacy.
Uses appropriate and safe strategies for crisis prevention intervention and follow-up.
Completes necessary program documentation in efficient and timely manner.
Ensures that program remains in compliance with HCBS Waiver regulations as well as agency philosophy and policies.
Performs other duties as assigned
Qualifications :
REQUIRED QUALIFICATIONS
Minimum of Associates Degree plus 2 years related experience for provision of Pre-Vocational services.
Knowledge of HCBS Waiver and ability to keep up-to-date on waiver program rules and regulations.
Understanding of Person Centered Planning.
Reasonable flexibility in work hours and ability to respond to scheduling needs of individual served.
Ability to establish and maintain productive working relationship with program participants families staff and community agencies.
Ability to effectively communicate both verbally and in written form.
Ability to organize work with self and others.
Ability to problem-solve and make judicious decisions regarding health and well-being of individuals supported.
May be required to lift in context of transferring individuals in/out of wheelchairs.
