AHRC New York City is seeking a Senior Case Manager for their Staten Island Advance & Earn Program.

Scope of Position:
The Senior Case Manager (SCM) oversees the overall coordination and direction of services for the people enrolled in services. The SCM monitors the people supported and ensures they remain focused and support them to successfully achieve goals and complete program requirements. The SCM helps assigned people enrolled to obtain additional resources intervenes in crises; and provides person-centered support and services and facilitates the learning process. They maintain each persons records by reviewing case notes logging events and progress. The SCM is expected to develop and maintain positive relationships with funding sources fellow providers schools training sites employers the people we support and their families and staff. The SCM is responsible for meeting the requirements of all contracts to which they are assigned and to adhere to all compliance requirements.
Essential Responsibilities:
- Promote program engagement track attendance and regularly assess training results of the people supported.
- Provide support with obtaining industry certification.
- Provide feedback for referral agencies.
- Participate as a member of the interdisciplinary team attending case conferences staff meetings and parent meetings as appropriate.
- Create and maintain excellent relationships with all the parties involved in the program.
- Create and maintain excellent relationships with all the people supported to monitor their progress and address their concerns in a timely fashion.
- Provide coverage within the program in case of excessive staff absences.
- Counsel students in both group and individual settings.
- Ensure the program remains in compliance with New York City and New York State regulations Department of Labor regulations and OPWDD.
- Provide top-quality customer service to people supported and their families in coordination with the funding and the referral agencies.
- Conduct personal and group meetings with the people supported and their network of supports.
- Establish relationships in community and coordinate outreach events for recruitment of new enrollees into programs.
- Conduct reviews of the expectations of the program and provide guidance to people supported family and other service providers involved.
- Perform Employment Discovery process for the people we support when required.
- Participate in Person Centered practices including Discovery Community Engagement activities and Customized Employment.
- Deliver Work Readiness Occupational Training and other program related curriculum when needed.
- Perform other duties as assigned.
Qualifications :
Required Qualifications
- Bachelors degree required Masters degree a plus.
- Two years of working with the developmentally disabled population preferred.
- Ability to communicate effectively both verbally and in written form.
- Ability to establish and maintain effective working relationships with participants families employers funding sources Care Coordinators and other stakeholders.
- Ability to organize work of self and others as well as work independently as needed.
- Experience or strong capacity in instructing/mentoring program participants with prior work exposure in teaching or training individuals highly desirable.
- NYS Drivers License preferred.
