Registered Nurse (RN) full time Days ICF setting

At our behavioral health center (an Independent Care Facility or ICF) Nurses play a key role in helping our residents feel safe respected and supported as they work toward their clinical and recovery goals. Youll be part of a close-knit multidisciplinary team that values compassion collaboration and stability in care.

In this role you will:

• Provide safe compassionate nursing care in a behavioral health setting
• Administer medications and treatments in accordance with individualized care plans
• Observe resident progress and respond thoughtfully to changes in condition
• Support and guide CNAs while working together as a unified care team
• Help maintain a calm structured and therapeutic environment using de-escalation skills when needed

Qualifications :

Floor Nurse:  We will consider a Registered Nurse (RN) or Licensed Practical Nurse (LPN) for this role.

What we need from you:

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in IL
• Prior floor nurse experience within a skilled nursing facility is strongly preferred
• Experience with the post-acute patients specifically the cardiac post op ortho & the medically complex
• Electronic medical records experience is helpful (Point Click Care or PCC is ideal)
• New Grads are encouraged to apply

This center is managed by TRANSITIONAL CARE MANAGEMENT.  We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs) Short-term Care Rehabilitation Intermediate Care (ICFs) and Behavioral Health Service (SMHRFs) centers.
