Job Summary
Position Summary
V- Tech Solutions is seeking a Care Manager II to support children and young adults with special needs by providing comprehensive care coordination services. The role focuses on assessment care planning service coordination and collaboration with families healthcare providers and community partners.
Key Responsibilities:
- Manage a caseload of members with moderate to complex needs
- Conduct assessments and community-based outreach visits
- Develop and monitor individualized care coordination plans
- Coordinate medical behavioral health and community services
- Support discharge planning and transitions of care
- Educate members and families
- Document care activities accurately
Qualifications:
- Masters Degree in Social Work or related field
- Active DC LICSW license
- 3 years care management or community health experience
- Strong communication and documentation skills
- Spanish preferred
