Your responsibilities

  • Carry out customer assessments and reviews in a timely personcentred way.
  • Support customers to regain or maintain independence.
  • Collaborate with a wide range of professionals and partners.
  • Contribute to a responsive system for managing urgent work.
  • Ensure all work is completed to a high standard with excellent recordkeeping.
  • Positively represent the Home First service and uphold Suffolk County Councils values.

You will need

  • Excellent communication skills with the ability to work effectively with customers their families advocates and other professionals.
  • Energy and adaptability particularly when managing urgent or changing priorities.
  • Resilience and a positive attitude even when working under pressure.
  • A genuine passion for helping people live safely independently and confidently.

The team

Home First is a supportive motivated and personcentred team working with adults to help them regain confidence and independence after a period of illness or difficulty. Made up of practitioners therapists and coordinators the team shares a genuine passion for empowering people to live well at home. The environment is fastpaced friendly and collaborative with colleagues always ready to help whether youre problemsolving a complex case celebrating a customers progress or responding to urgent need.
