Your Role: Home Health Occupational Therapist
Key Responsibilities:
As aHome Health Occupational Therapist youll help patients:
Regain independence through customized therapy plans
Improve functional abilities for activities of daily living (ADLs)
Recover from injury surgery or hospitalization in the comfort of home
Reduce fall risk and improve home safety
Your Day-to-Day Includes:
Performing in-home OT evaluations and reassessments
Creating individualized care plans tailored to each patients goals
Collaborating with nurses physical therapists and physicians
Educating patients and caregivers on proper techniques and safety
Documenting patient progress in compliance with state and Medicare guidelines
What Were Looking For:
Arizona-licensed Occupational Therapist (OT)
CPR certification (AHA-approved)
At least 1 year of clinical experience (Home Health experience preferred)
Organized compassionate and reliable
Valid drivers license auto insurance and reliable transportation
Strong communication skills and clinical decision-making
