Job Description:

JOB/RESPONSIBILITY DESCRIPTION

DAY PROGRAM INSTRUCTOR - TEAM LEADER

The Salvation Army Broadview Village supports individuals with developmental disabilities by

providing a Christian lifestyle in community based living and day program environments.

Statement of Purpose:

To provide administrative support to the Day Program Manager and to provide support and advocacy for residents/participants in meeting their goals and needs

Accountable to:

Day Program Manager/Supervisor or designate

Qualifications:

Completed Developmental Services Worker diploma or other degree/diploma related to human services

Three (3) years full-time experience working with people who have developmental disabilities

Demonstrated ability and experience in Individual Support Planning Experience in program (teaching/training plans or behavior management programs and reinforcement schedules

Teaching skills (demonstrated ability to implement various teaching approaches and/or group instructor skills)

Proficient in medication administration

Valid Standard First Aid certificate (including CPR) Nonviolent crisis intervention and behaviour management skills.

Computer skills (Microsoft Office internet)

Training and/or experience in personal care (i.e. assisting with G-tube feedings continence devices) is an asset and willingness to be trained

Training and/or experience in supporting individuals through ambulates mobilizers transfers is an asset

Valid drivers license is an asset

Current clear Vulnerable Sector Check acceptable to the Salvation Army

RESPONSIBILITIES

A PROGRAM

Know and respond to safety and health care needs of residents/participants

Act as a role model for all residents/participants as reflected by appearance behaviour and attitude

Perform Prime Worker/Caseload duties as assigned by Residential/Day Program Manager/Supervisor

Provide support for residents/participants personal development as decided through Individual Support Planning

Supervise and assist residents/participants with meal preparation and clean-up

Administer medication as prescribed and according to policies and procedures

Support resident/participant involvement in activities (Faith Finders bowling special events etc.)

Complete Shift Reports/Daily Logs and review/update

Communication Book Maintain pertinent resident/participant records

Support family involvement with residents/participants site activities and special events

Follow and promote the agencys and site program goals and philosophy

Liaise with transportation companies day program staff group home staff other professionals families etc.

Ensure learning opportunities are provided using agency and community resources

B STAFF

Maintain effective communication

Assist in orientation as directed by Residential/Day Program Manager/Supervisor.

Provide support and guidance to coworkers placement students volunteers and/or external agency staff

Represent the site on committees and at other meetings as delegated by the Residential/Day Program Manager/Supervisor

Attend and participate in meetings and staff development training at the discretion of the Residential/Day Program Manager/Supervisor and Administration Counsellor-In-Charge (CIC) in the absence of Residential/Day Program Manager/Supervisor (see CIC description below)

C

HOUSEHOLD ORGANIZATION

Adhere to site menu plan and food restrictions

Ensure the general site appearance is clean and tidy before the shift is completed

Maintain and account for use of site and resident monies TTC tokens taxi chits household supplies etc.

Complete daily household routines as required (laundry cooking cleaning etc.)

HEALTH & SAFETY

Comply with the provisions of the Occupational Health & Safety Act and regulations

Adhere to the agencys Health & Safety policies and program

Complete daily walkthroughs/inspections of site as assigned

Report complete and forward maintenance requisition forms to main office

Report accidents/injuries/workplace hazards in a timely manner

Responsible use of the Agencys vehicles equipment and property

Follow the principles of nonviolent crisis intervention philosophy

E

ADMINISTRATIVE

Assist in completion of staff pay sheets/Work Day as assigned by Day Program Manager/Supervisor

Assist in recording vacation and sick accruals/use as assigned by Residential/Day Program Manager/Supervisor

Scheduling in consultation with Residential/Day Program Manager/Supervisor

Update Rotational Relief list

Ensure household/program inventory is maintained in consultation with the Residential/Day Program Manager/Supervisor

Ensure supply of TTC tokens taxi chits office and medical supplies etc.

F

ORGANIZATIONAL

Adhere to standards that are consistent with the values and philosophy of The Salvation Army and Broadview Village

Be familiar with and adhere to Salvation Army policies and procedures

Participate fully and constructively in meetings and in-service training as required

Participate in regular PEAC coaching/supervision and consultation with supervisor

Participate in formal PEAC annual review sessions with supervisor

As required and is applicable make suggestions that will improve efficiency working conditions or procedures to management

Represent the organization in a professional manner

Other duties as assigned.

The position of Team Leader encompasses the designation of Counselor-In-Charge (CIC) in the absence of the Residential/Day Program Manager/Supervisor as described below:

Statement of Purpose:

The role of the C.I.C. is to oversee the program needs in the absence of the Day Program Manager/Supervisor

A

SHIFT

Direct delegate coordinate organize and ensure the completion of shift responsibilities

Ensure relevant information is communicated

Ensure adequate staff coverage to meet the needs of the the event of staffing shortage C.I.C. is responsible for the coverage of the succeeding shift until back up staff can arrive.

Ensure daily logs and program data are completed.

Ensure incident reports are completed and forwarded to main office

Ensure emergency procedures are followed

Ensure that medications Policies and Procedures are followed

B

HEALTH & SAFETY

1) Comply with the provisions of the Occupational Health & Safety Act and regulations

2) Adhere to the agencys Health & Safety policies and procedures

3) Ensure the completion of daily walkthroughs/inspections of site

4) Report accidents/injuries/workplace hazards in a timely manner

The position of Team Leader encompasses Prime Worker/Caseload duties for assigned residents/participants:

A

Attendance and participation in all program meetings and/or agency events as applicable

Team Meetings Admission/Transfer/Discharge meetings Annual Open House Individual Support Plan Meetings Program Review Meetings (monthly) / PA Days

Consultations with other professionals

Other events and/or meetings as assigned

B

Completion and presentation of information for program related meetings

Assessments Program Reviews and Data Summaries including graphs Individual Program Plan conference package

Reports applications and consents as required

Other documentation as required

C

Development implementation and evaluation of the Individual Support Plan (ISP) / individualized programming in consultation with the resident participant Residential/Day Program Manager/Supervisor the site program team and other members of the ISP Planning Team

D

Ensure the following for residents/participants as applicable

Health Care appointments in accordance with Ministry and agency standards Leisure and vacation activities in accordance with resident/participant preferences and needs The purchase suitability and maintenance of residents clothing A current inventory of residents belongings Responsible use and documentation of residents finances Support and/or coordination of day activities Relationships with family residential programs day programs peers friends volunteers and others Support and coordination of outreach and vocational opportunities Representation/advocacy for monthly planning (i.e. BVEP calendar of events)

PHYSICAL EFFORT/DEXTERITY REQUIRED: The performance of the job requires bending/twisting/lifting; considerable walking; standing; writing; computer work; and light housekeeping.

WORK ENVIRONMENT: The incumbents work environment is typically in a group home or day program/classroom type facility.

The incumbent works under generally agreeable conditions with the following exceptions:

  • Exposure to dirt bodily fluids bodily waste
  • Noise
  • Dealing with angry and abusive people
  • Verbal abuse
  • Handling garbage
  • Odours

EXPOSURE TO HAZARDS: The incumbent may be exposed to risk of minor to moderate injury or illnesses due to the following:

  • Physical attacks by others (i.e. being attacked by a participant)
  • Exposure to infectious diseases
  • Exposure to toxic chemicals
  • Exposure to bodily fluids
  • Sharp objects
  • Other dangerous situations

PRECAUTIONARY MEASURES: The incumbent must take the following precautionary measures (in addition to Health and Safety rules):

  • Universal Precautions
  • WHMIS Training
  • First Aid certification
  • Police/Criminal Reference Check Vulnerable Sector Screening
  • Nonviolent crisis intervention training

Work Schedule: 40 hours per week Monday-Friday 7am-3pm

Main location is 150 Railside Rd occasionally may work at 1132 Broadview Ave. - to meet program needs

Compensation:

The target hiring range for this position is $28.27 to $29.31 with a maximum of $30.35.

Placement in the salary range will be based on factors such as market conditions internal equity and candidate experience skills and qualifications relevant to the role.
