Position Summary
The Physical Therapist provides high-quality patient-centered therapeutic care by managing the full rehabilitation process for an assigned caseload of patients. This role works collaboratively with physicians and interdisciplinary care teams to evaluate patient needs develop individualized plans of care and deliver effective therapy interventions focused on functional improvement patient engagement and positive clinical outcomes.
Responsibilities
Perform patient evaluations and deliver treatment interventions within the scope of practice and professional standards.
Assess therapeutic rehabilitative and functional status and contribute to the development of individualized care plans.
Provide skilled physical therapy services using evidence-based procedures and techniques aligned with the physicians plan of care.
Educate patients families and caregivers on therapeutic exercises adaptive techniques and use of assistive devices.
Manage and prioritize caseloads while coordinating daily assignments and care handoffs.
Support patients throughout the continuum of care to maximize recovery and functional independence.
Required Qualifications
Bachelors degree in Physical Therapy (required)
Active State Physical Therapist license
BLS certification
New graduates are encouraged to apply (prior experience not required)
