Position: Nurse Practitioner (Hybrid)
Schedule: MondayFriday 8:00 AM5:00 PM
Hybrid Role: 12 days telehealth (remote) 34 days in patients homes
Compensation: Base salary 15% bonus

Overview
We are seeking experienced Nurse Practitioners and Physician Assistants to join an integrated care team focused on improving outcomes for patients with chronic illness including kidney disease. This hybrid role blends in-home visits with telehealth allowing providers to deliver personalized evidence-based care where it matters most.

Youll work closely with a Registered Nurse and Social Worker to improve quality of life slow disease progression and reduce avoidable hospitalizations.

Key Responsibilities

Conduct comprehensive assessments and annual wellness visits (home and virtual)
Develop implement and adjust treatment plans
Prescribe medications and order diagnostic testing
Coordinate care with PCPs specialists and ancillary services
Educate patients and families on disease management and preventive care
Maintain accurate timely clinical documentation
Participate in integrated care team meetings
Follow up and evaluate patient progress

Requirements

Active unrestricted NP or PA license
Board certification (NP: ANCC/AANP PA: NCCPA)
Active DEA license
2 years of direct patient care experience
Experience with annual wellness visits (home or virtual preferred)
Ability to practice autonomously
Valid drivers license reliable transportation and insurance
Managed Care / IPA / Health Plan experience preferred
