Position Summary
Team Leader Aged Care and Disability Services
THIS IS A FIXED TERM CONTRACTUAL ROLE WITH A SCOPE FOR EXTENSION
Under the supervision and direction of the Aged Care and Disabilities Program Manager the Team Leader is responsible for supporting the effective and efficient coordination and delivery of Aged Care and Disability services at MDAS.
The Team Leader works collaboratively within the team on a daytoday basis and is responsible for ensuring a responsive and culturally appropriate operational service delivery framework that meets performance targets and community needs as identified in the MDAS Strategic Plan.
Key functions include:
- Allocation of referred clients to the appropriate Aged Care and Disability programs or other MDAS supports
- Leadership and coordination of Social Support Group activities
- Supporting staff to deliver services aligned with client needs
Key Responsibilities
Service Delivery and Development
- Demonstrate practical and innovative leadership in the implementation monitoring and evaluation of programs for clients.
- Complete accurate and timely case notes and client documentation.
- Maintain daily weekly and monthly service delivery records in relevant data management systems.
- Liaise with relevant professionals to support effective care planning and service delivery.
- Undertake other duties as required.
Leadership and Management
- Actively supervise staff to ensure quality and culturally safe client care.
- Manage conflict resolution within the Aged Care and Disability Services.
- Provide regular reporting and feedback to the Program Manager.
- Oversee the delivery of Social Support Groups.
- Facilitate regular team meetings.
- Prepare required reports for funding bodies and management.
Relationship Management
- Foster and maintain effective relationships with internal and external agencies.
- Attend relevant meetings forums and education programs to ensure contemporary practice.
- Work collaboratively to streamline intake assessment and referral pathways.
- Monitor and support partnerships that enhance client and community wellbeing.
SelfManagement and Team Contribution
- Maintain confidentiality in line with the MDAS Code of Conduct and policies.
- Escalate concerns that may place clients or families at risk through appropriate reporting pathways.
- Contribute to policy and procedure development with a focus on continuous improvement.
- Comply with MDAS corporate policies and State/Commonwealth guidelines.

Demonstrated client centred practice with an interest in clients needs.
Experience in a multidisciplinary health care environment coordinating and delivering services

Strong verbal and written communication skills including the ability to interface and coordinate work efficiently and effectively with both external and internal customers.

Qualification/s:
Relevant experience and qualification in a health related field such as Diploma Community Services or Aged Care.
Other Compliance requirements
A current Drivers License that allows you to drive in Australia.
A current Employee Working with Children Check card.
A current National Police Check.
