The Role
As a Primary Care Physician youll serve as the central provider for your patient panel delivering high-quality comprehensive care in home-based and community settings supported by an experienced interdisciplinary team.
Your Impact Will Include:
- Providing preventive acute and chronic care for senior patients
- Conducting comprehensive health assessments and managing personalized care plans
- Ordering and interpreting labs and diagnostics
- Collaborating closely with Nurse Practitioners Physician Assistants and Wellness Coordinators
- Leading population health and quality improvement initiatives
- Mentoring and supporting advanced practice providers
- Participating in provider meetings and continuing clinical development
What Were Looking For
- MD or DO with an active California medical license
- Board Certified or Board Eligible in Family Medicine Internal Medicine or Geriatrics
- Experience with geriatric or senior-focused care (strongly preferred)
- Comfortable with EMR systems and team-based care models
- Compassionate communicative and patient-centered
