RN- HospitalCare Manager- Up to $$74.30/ hour

Schedule:Full-Time M-F 8am - 4:30pm
Pay rate: Up to $74.20 depending on experience
Position ResponsibilitiesInclude:
- Conducts thorough and accurate psycho-social assessments of patients in an inpatient hospital unitin a timely manner with a patient centered approach.
- Actively participates on interdisciplinary team to develop and implement care plans during hospitalization.
- Establishes and implements care plans to ensure safety continuity of care appropriate care coordination and support post hospitalization.
- Uses knowledge of community resources healthcare funding along with county state and federal regulations in making appropriate and safe discharge plan.
- Communicates promptly clearly respectfully and effectively with care team patient and support system. Coordinates and attends care conferences as applicable.
- Documents work clearly concisely and consistently in a timely manner.
- Works closely with the Social Worker throughout hospitalization and during discharge planning.
Required Skills/Qualifications:
- An active RN license in the states of MN.
- BSN is preferred but not required.years of current RN experience is preferred.
- One year of hospital Case Management experience is preferred.
- Current BLS/ CPR
- Ability to pass a criminal background check.
