As we grow across the greater San Diego metro area were seeking Primary Care Physicians (MD/DO) who want to practice relationshipcentered medicine within a supportive physicianled organization that invests in longterm success.

The Role

Youll serve as the primary physician for a panel of senior patients delivering coordinated care across homebased and community settings. Youll be supported by an established interdisciplinary team and clear clinical playbooksso you can focus on highquality care not fragmentation.

Your Responsibilities Include:

• Providing preventive acute and chronic care for older adults
• Conducting indepth assessments and managing individualized care plans
• Ordering and interpreting labs and diagnostics
• Collaborating closely with Nurse Practitioners Physician Assistants behavioral health and palliative partners
• Participating in population health and quality improvement initiatives
• Supporting and mentoring advanced practice providers within your pod
• Engaging in ongoing clinical collaboration coaching and professional development

Who Thrives Here

• MD or DO with an active California medical license
• Board Certified or Board Eligible in Family Medicine Internal Medicine or Geriatrics
• Experience caring for seniors or medically complex patients (preferred)
• Comfortable practicing in a teambased homecentric care model
