Role Overview
We are seeking a Licensed Practical Nurse (LPN) to support a Long Term Care (LTC) setting. The LPN will provide direct nursing care under the supervision of a Registered Nurse physician or other licensed healthcare provider. This role focuses on maintaining resident health administering treatments and supporting individualized care plans in a structured care environment.
Key Responsibilities
Provide nursing care consistent with established treatment plans and scope of practice.
Assist Registered Nurses physicians and licensed healthcare providers in identifying resident needs and developing care plans.
Administer prescribed medications and perform nursing procedures according to facility policies.
Obtain and accurately record vital signs including temperature blood pressure pulse and respirations.
Observe monitor and document residents physical and behavioral conditions.
Identify and report changes in health status or responses to medications promptly to supervising clinicians.
Support resident comfort safety and well-being while promoting dignity and quality of life.
Maintain accurate clinical documentation in accordance with regulatory and facility standards.
Adhere to infection control safety and compliance guidelines at all times.
Qualifications
Active Licensed Practical Nurse (LPN) license with current registration in New York State.
Ability to work effectively under the direction of a Registered Nurse or licensed provider.
Strong clinical observation documentation and communication skills.
Knowledge of nursing principles long-term care practices and patient safety standards.
Ability to follow established procedures and respond appropriately to changes in resident condition.
