Job Description:

Sickle Cell Disease Program Manager

Southern Jersey Family Medical Centers Inc. is looking for a talented Sickle Cell Disease Program Manager with the highest level of professionalism to step into our fast-paced facility!

Summary:The Sickle Cell Disease (SCD) Program Manager oversees and coordinates comprehensive patient care managing multidisciplinary teams developing strategic plans engaging communities and ensuring high-quality equitable services for SCD patients across the lifespan involving clinical operations data analysis advocacy and stakeholder liaison.

Hours: This is a full-time position that requires the ability to work a flexible schedule including weekends as is a grant position.

Position Responsibilities/Duties:

Develops vision strategy and operational plans for the SCD program aligning with grant and organizational goals.
Manages care across all settings (inpatient outpatient) acts as a liaison and ensures seamless whole-person care.
Leads SCD Pilot Program team fosters collaboration (hematology pain social work) builds partnerships with community groups government and donors.
Designs implements and oversees community engagement strategies. Works collaboratively with marketing to create educational materials.
Tracks program metrics analyzes SCD data prepares reports for leadership and state grant requirements and ensures regulatory compliance.
Excellent verbal and written communications skills
Other duties as assigned

Experience and Skills:

Bachelors degree in social work public health or related field required.
Background in hematology chronic disease management social work or public health.
Strong project management leadership and coordination skills.
Excellent written/verbal skills for diverse audiences (patients clinicians policymakers).
Ability to interpret and present data to for quarterly reporting and project success
Dedication to improving lives of people with SCD
Valid NJ Drivers License and reliable transportation
