Your Position at St John of God Berwick Hospital
You will work alongside the multi-disciplinary team to provide senior social work services to clients/patients.
We are seeking an experienced casual Social Worker to provide weekday leave cover across our inpatient and outpatient rehabilitation departments and within our acute medical and surgical wards. Primary role is to support the multi-disciplinary team with complex discharge planning within populations that may include but not limited to; chronic illness oncology and orthopaedics. There may also be opportunities to assist with running group education sessions to our outpatient population within the cardiac pulmonary and oncology rehabilitation programs.
The Position
Demonstrated clinical knowledge and skills in assessment planning implementing and evaluating Social Work interventions.
Provide Social work services to DVA TAC and Workcover patients.
Actively intervene with appropriate counseling support education community liaison and referrals Actively participate and where appropriate lead family meetings.
Participate in multi-disciplinary service approach for clients including contributing to the evaluation of the Social Work service and Clinical Specialty.
Deliver patient-centered care building health literacy and promoting self management. Demonstrate reflective practice and commitment to professional development.
Supports Social Work service by communicating issues and ideas for enhancing the service. Supports the profession through Social Work student learning.
Liaise with compensable bodies financial institutions and external care agencies where appropriate Liaise with patients and their Carers to identify social support requirements Contribute to effective and efficient patient discharge planning via timely collaboration and communication with the multi-disciplinary team the patient and their Carers.
You will have a tertiary qualification in Social Work and eligible for membership with the Australian Association of Social Work. You willdemonstrate skills and experience in social work assessments case management and discharge planning.
To succeed you will demonstrate previous capacity to collaborate within a multi-disciplinary team.

The Benefits
Salary: $46 to $63 per hour (casual loading included) plus 12% superannuation
