Duties

Monitor patient flow and discharge readiness
Perform utilization reviews
Coordinate post-acute services
Collaborate with clinical teams
Maintain compliance documentation

Qualifications

NY RN license
Minimum 5 years RN experience
Care management experience required
ASN/ADN
BLS

Preferred

BSN/MSN
Case Management background
