About the Role We are seeking a bilingual Clinical Health Coach (Spanish and English) with a clinical background such as Registered Dietitian Registered Nurse or this virtual role you will engage with patients weekly support healthy behavior change and collaborate closely with an interdisciplinary care team led by obesity medicine expert Dr. Jamie Kane. This position offers flexible scheduling remote work and opportunities for professional growth in a rapidly evolving field.

Key Responsibilities

• Conduct weekly virtual checkins to monitor progress and support care plan adjustments
• Serve as the primary communicator for patient questions and concerns
• Act as a liaison between patients and the broader clinical team
• Provide practical guidance on nutrition lifestyle habits and behavior change strategies
• Collaborate with physicians nurse practitioners dietitians and fellow health coaches
• Document patient interactions and updates in electronic health record systems
• Use telehealth platforms and digital tools to support remote care delivery

Qualifications

• Bachelors degree in Health Education Nutrition Psychology Public Health or a related field preferred
• Health Coaching certification from a recognized program required
• Minimum of 1 year of experience as a Health Coach Patient Navigator or similar role required
• Experience in obesity care weight management chronic care management or telehealth preferred
• Strong communication skills including motivational interviewing
• Proficiency with electronic health records and Google Workspace
• Reliable internet connection and a HIPAAcompliant private workspace
• Bilingual in Spanish and English required
