Liquid Personnel is recruiting anAdult Social Worker to join its clients Learning Disabilities team based in Cardiff.
Supporting adults with learning disabilities to live fulfilling safe and independent lives. The position focuses on strengthsbased practice outcomefocused planning and statutory responsibilities under theSocial Services and Wellbeing (Wales) Act 2014. You will work collaboratively with individuals families carers and partner agencies to assess needs develop care and support plans and ensure people receive the right support at the right time.
What will your responsibilities be
- Undertake holistic assessmentsof adults with learning disabilities ensuring all work aligns with the Social Services and Wellbeing (Wales) Act 2014 and promotes independence choice and control.
- Develop and review outcomefocused care and support plansthat reflect the individuals strengths aspirations and personal outcomes.
- Work collaboratively with multidisciplinary professionalsincluding health colleagues education providers housing teams and voluntary sector organisations.
- Support safeguarding processesby identifying reporting and responding to concerns in line with Wales Safeguarding Procedures.
- Undertake Mental Capacity Act assessmentsand contribute to bestinterest decisionmaking where required.
- Provide professional advice and advocacyto ensure individuals rights wishes and wellbeing remain central to all planning and intervention.
- Coordinate transitionsfor young people moving from childrens services into adult learning disability services.
- Maintain accurate timely case recordsand produce highquality reports in line with organisational and legislative standards.
- Contribute to service development quality assurance and continuous improvement initiatives within the Learning Disabilities service.
Qualifications and Experience:
To be successful in this role you must have;
