About the role
As a Clinical Care Coordinator youll lead our clinical and care teams to ensure residents receive exceptional care while nurturing an understanding of their unique identities and experiences. Youll also play a pivotal role in our homes clinical governance and may manage specialized clinical portfolios such as education palliative care and infection control.
Your main duties will include:
- Ensure comprehensive clinical and specialized nursing care is provided documented and tailored to each residents needs by our clinical staff.
- Prioritize residents and their families in decision-making considering their preferences independence dignity and identified care-related risks.
- Oversee and conduct all scheduled AN-ACC reappraisals including mandatory and voluntary assessments as required.
- Hold Registered Nurses (RNs) and Enrolled Nurses (ENs) accountable for daily case reporting care planning and addressing patient concerns promptly.
- Conduct routine clinical assessments to promptly address identified gaps in care.
About you:
- Currently registered as a Nurse with AHPRA with no restrictions.
- Demonstrated management experience in aged care sector
- In-depth knowledge of the aged care industry with a keen eye for emerging trends and best practices.
- Proficiency in managing human resource matters complemented by strong negotiation skills.
- Skilled in utilizing various information technology tools and software including Word and Excel with a quick ability to adapt to new skills as required.
