Job Description:
Our client is actively resourcing for 3x Adult Social Workers to support the following teams in Bridgend. CMHT West Network Team (Community) and Hospital
The postholder will undertake complex assessments coordinate care and support under the Mental Health Act 1983Mental Capacity Act 2005 and theSocial Services and Wellbeing (Wales) Act 2014.
What will your responsibilities be
Assessment Care Planning and Review
- Undertake strengthsbased personcentred assessments under the Social Services and Wellbeing (Wales) Act 2014 identifying eligible care and support needs for adults and carers
- Develop implement and review care and support plans that promote independence wellbeing and community inclusion
- Complete assessments relating to hospital admission discharge planning community living and mental health needs
- Support individuals to access appropriate services including domiciliary care reablement residential care equipment adaptations and direct payments
Safeguarding and Risk Management
- Identify assess and respond to safeguarding concerns including abuse neglect selfneglect and exploitation
- Undertake safeguarding enquiries and contribute to protection planning in line with statutory safeguarding procedures
- Assess and manage risk working collaboratively with individuals families and partner agencies
- Participate in multiagency safeguarding meetings case conferences and risk management forums
Mental Health and Specialist Practice
- Work with adults experiencing mental health difficulties contributing to assessment care planning and recoveryfocused intervention
- Complete or contribute to Care and Treatment Plans under the Mental Health (Wales) Measure 2010
- Work in partnership with health professionals within multidisciplinary teams
- Where qualified undertake duties as an Approved Mental Health Professional (AMHP) and contribute to Mental Health Act assessments
Mental Capacity and Legal Responsibilities
- Assess mental capacity in accordance with the Mental Capacity Act 2005
- Make and record bestinterest decisions and coordinate bestinterest meetings
- Undertake or support Deprivation of Liberty Safeguards (DoLS) applications and reviews
- Ensure practice is legally sound rightsbased and proportionate
Hospital Social Work and Discharge Planning
- Contribute to or lead safe and timely discharge planning from hospital settings
- Undertake urgent assessments to support discharge and prevent delayed transfers of care
- Work closely with ward staff discharge coordinators and community teams to ensure continuity of care
- Address complex discharge scenarios involving capacity safeguarding housing or ongoing care needs
Partnership and MultiAgency Working
- Work collaboratively with health services housing education police voluntary and independent sector partners
- Attend and contribute to multidisciplinary meetings reviews and panels
- Support carers families and informal networks with advice guidance and signposting
- Promote preventative approaches and early intervention wherever possible
Professional Practice and Development
- Maintain accurate timely and highquality case records in accordance with Council policies and professional standards
- Work in line with Social Care Wales Codes of Professional Practice
- Participate in supervision training continuing professional development and service improvement activity
- Support students newly qualified social workers or colleagues as appropriate
- Uphold principles of equality diversity dignity and human rights in all areas of practice
Qualifications & Experience:
To be successful in this role you must have
- Professional Qualification:MA BA BSc in Social Work Diploma in Social Work or CQSW.
- Registration:Must be registered withSocial Care Wales.
- Experience: 2 years post-qualified experience working in a statutory setting.
- Legislation Knowledge:Strong understanding of theMental Health Act 1983Mental Capacity Act 2005Social Services and Wellbeing (Wales) Act 2014 safeguarding procedures and the Mental Health (Wales) Measure 2010.
- Skills:Excellent assessment communication and riskmanagement skills with the ability to work effectively in a fastpaced multidisciplinary environment.
- Driving Licence:Full UK driving licence and access to a vehicle is often required for community visits.
