Position Summary
The Community Living Supervisor is responsible for overseeing the daily operations of assigned Community Homes ensuring the health safety and well-being of individuals with intellectual or developmental disabilities. This role involves providing direct care supervising staff and implementing outcome plans to help individuals achieve their personal goals. The supervisor ensures compliance with all regulatory and organizational policies.

  • Enroll in the Front Line Supervisor (FLS) certification program.
  • Acquire FLS badge/certification within 1 year of enrollment.
  • Receive a pay rate increase ($1/hr) and one-time bonus upon completion.

Key Responsibilities

  • Provide direct care support to people with intellectual or developmental disabilities and specialized care in daily living social and recreational activities.
  • Ensure compliance with the Departments Policies and Procedures relative to Community Homes.
  • Assemble maintain and monitor all in-house and personal records.
  • Assist with and ensure the upkeep and maintenance of the home.
  • Implement formal and informal outcome plans to assist the person in achieving their personal outcomes.
  • Develop the weekly staff schedule and adjust staff schedules due to vacation absenteeism or illness with prior approval.
  • Communicate and resolve program and individual concerns with the Community Homes Supervisor on a regular basis.
  • Assign and ensure completion of each persons Annual Assessment and supporting documents.
  • Distribute individual funds acquire and submit receipts and assist with banking as advised.
  • Coordinate medications and ensure all prescribed medication is dispensed and detailed per policy (training provided).
  • Performs other related duties as assigned.



Qualifications
  • Educational Requirement: Equivalent to a Bachelors Degree and/or 2 years of experience working directly with individuals with an intellectual disability is preferred.
  • Flexibility: Must be flexible to work on-call as needed.
  • Preferred Certification: An accredited certification as a Nursing Assistant (CNA) is highly preferred.
  • Physical Requirements: Ability to complete one-person transfers is essential.
  • Supervisory Experience: Supervisory experience is preferred.
  • Clearance Requirements: Must meet Act 33 and 34 Requirements.
  • Certification Requirements: Must obtain First Aid/CPR certification within two months of start date and successfully complete Medication Administration training.
  • License & Travel: Access to transportation and a valid drivers license are necessary for local travel.
