ABOUT US
We are a mission-driven hospice committed to providing exceptional comfort support and dignity to patients and families. We are expanding our outreach efforts and seeking a compassionate motivated Community Liaison to help educate the community and build meaningful relationships with referral partners.

POSITION SUMMARY
The Community Liaison is responsible for increasing awareness of hospice services by developing strong relationships with skilled nursing facilities assisted living communities physician offices and other community partners. This role focuses on education relationship building and helping ensure patients receive timely appropriate hospice care.

RESPONSIBILITIES
Educate healthcare professionals and community groups on hospice eligibility and services.
Maintain a consistent weekly cadence of in-person visits.
Develop and strengthen referral relationships with SNFs ALFs physician offices and community organizations.
Provide value-based tools and resources to help partners identify patients who may benefit from hospice.
Communicate regularly with the admissions and clinical teams to ensure smooth coordination for referred patients.
Track activity referrals and growth in assigned territory.
Represent the hospice at community events and educational programs.

QUALIFICATIONS
Experience in healthcare marketing senior care hospice home health or related field preferred.
Strong relationship building and communication skills.
Ability to work independently and manage a territory effectively.
Knowledge of hospice philosophy and eligibility preferred (training available).
Reliable transportation and valid drivers license.
Compassionate energetic and professional demeanor.
