JOB SUMMARY:

This role is supported by time-limited funding. While continuation beyond the current funding period is not guaranteed the organization is committed to pursuing ongoing funding opportunities.


The Family Resource Navigator will identify and connect with community resources and viable coalitions as well as civic community health and human services partners within the community.

These efforts will be the foundation for creating a Family Resource Center (FRC) at PCCT to serve agency participants and community members. This will be accomplished through two activity categories - Family Management and Referral Systems Navigation.

The Family Resource Navigator will establish and maintain communication with community resources to support positive relationships for families with children and ensure strong referral pathways to and from our programs. The Family Resource Navigator will also establish new community resource partners to strengthen referral pathways between agencies work with families to ensure their needs are met coordinate training and systems orientation opportunities as well as distribute temporary emergency supplies until referral to other agencies/programs occur.

PRIMARY RESPONSIBILITIES:

  1. Offer rapid response (e.g. a phone call within 48 to 72 hours) when a family has requested assistance.
  • Listen and provide confidential emotional support.
  • Help families with goal-setting.
  • Empower families to make informed choices that are right for them.
  • Assess families needs and strengths.
  • Collect assessment and referral information.
  • Provide peer support in one-on-one and group settings.
  • Role model and coach families how to identify and access the supports they need.
  • Refer and link families to community resources.
  • Help families understand the mental health evaluation diagnosis and treatment process.
  • Discuss options in an unbiased and culturally sensitive manner.
  • Explain processes rights and responsibilities and timelines for services.
  • Offer guidance with forms and paperwork.
  • Support transition safety and reentry planning.
  • Identify relevant benefits programs (e.g. Medicaid SNAP); support enrollment as appropriate.
  1. In collaboration with the Community Alignment Manager and Prevention Community Educator engage in school or community outreach and awareness events.
  2. Identify and nurture relationships with community resources services and providers that can offer system support to families in a timely and appropriate manner as referred by PCCT. Maintain an updated list of community resources to support PCCT Family Connects and other PCCT staff as they connect families with resources as needed.
  3. Coordinate FRC meetings workshops and trainings (e.g. parent information programs parent education family health screenings etc.) for the purpose of delivering services in compliance with established guidelines.
  4. Responsible for the purchase and inventory of consumables and supplies to keep the FRC stocked and organized.
  5. Work with the Community Alignment Manager to promote the Family Resource Center/PCCT by communicating with referral sources (e.g. ob/gyn practitioners hospitals pediatricians) communicating with groups that interact with families (e.g. employers newspapers churches childcare agencies the public) and soliciting support from community leaders (e.g. to publicize the program and to provide support for families).
  6. Create and Liaison with a Parent Advisory Committee for the Family Resource Center creating a forum for dialog ongoing needs assessment to identify priority needs of families and continuous quality improvement.
  7. Attend trainings as required to include but not limited to Standards of Quality for Family Strengthening & Support Developing and Sustaining Effective PACs and technical sessions.
  8. Demonstrates the agencys values of compassion respect learning adaptability and achievement.
  9. Other duties as assigned by the Community Alignment Manager.
Requirements

REQUIRED QUALIFICATIONS:

  1. Firsthand experience navigating systems on behalf of child youth or young adult with health challenges and working with families.
  2. Familiarity with the resources in your community and region.
  3. Understanding of relevant systems services and processes (e.g. TANF Medicaid enrollment).
  4. Strong interpersonal skills and empathy.
  5. Understanding of and ability to maintain family confidentiality.
  6. Familiarity and ability to work effectively with families of diverse racial ethnic socioeconomic and cultural (e.g. LGBTQ military) backgrounds.
  7. Strong written and spoken communication skills including familiarity with relevant languages for your region.
  8. Ability to collaborate and network with community organizations and groups.
  9. Active involvement in other community councils advisory boards or parent and family groups.
  10. Formal training and/or work experience within the social service/case management field or training specific to navigating community service systems).
  11. Ability to lift 25 lbs.

PREFERRED QUALIFICATIONS:

  1. Bachelors degree in the social services or other relevant field.
  2. Experience in human services in the public sector skill in interacting with human service providers and agency leaders and an understanding of the role of evaluation in social services.
