Our Intensive Care Coordination with Wraparound (ICCW) program supports children youth young adults and families with complex needs through a collaborative family-driven approach. This community-based role blends Wraparound planning with comprehensive care coordination allowing Care Coordinators to work directly with families and community partners to create meaningful individualized plans. This position requires a bachelors degree in a Human Service field.
