About the Role
As a Care Partner in our Support at Home team you will play a vital role in helping older people live independently and safely in their homes. Youll coordinate and monitor tailored aged care services working closely with participants families health professionals and support staff to ensure care is responsive respectful and aligned with individual needs and government requirements.

Key Responsibilities
- Develop and review individual care plans based on each participants needs and goals
- Coordinate service delivery with clinical allied health and support teams
- Monitor participant wellbeing and service outcomes
- Support participants and families to understand care options and make informed decisions
- Ensure compliance with aged care quality and safety standards
- Oversee participant budgets and ensure services are cost-effective and appropriate
What Youll Bring
- Certificate III in Individual Support (Ageing) or Certificate III in Health Services Assistance (plus experience)
- Current drivers licence and reliable vehicle
- Experience working with older people families and health professionals
- Knowledge of aged care compliance and quality standards
- Strong communication coordination and problem-solving skills
- A passion for empowering older people to live their best life
Desirable Qualifications (not essential)
- Certificate IV or Diploma in Aged Care Disability Community Services Nursing or Ageing Studies
