Role Overview
A community-based not-for-profit organisation is seeking an experienced Clinical Care Team Lead to provide day-to-day leadership clinical oversight and professional support to its Clinical Care team.

Working closely with the Executive Manager Service and Quality this role ensures the delivery of safe effective and person-centered clinical services across community and telehealth settings.

The Clinical Care Team Lead leads a multidisciplinary team of Registered Nurses (Clinical Care Advisors) and other clinical and allied health professionals. The role works in partnership with customers families carers primary health providers and internal teams to ensure strong clinical governance best-practice care and compliance with all relevant NSW and Australian legislation standards and guidelines.

This position plays a key role in strengthening clinical capability across the organisation driving continuous improvement improving clinical literacy and ensuring early identification and response to clinical risk deterioration and complex care needs.

Key Result Areas

Clinical Leadership & Team Management

  • Provide leadership supervision mentoring and coaching to the Clinical Care team.
  • Deliver regular clinical oversight case reviews and reflective practice.
  • Monitor performance productivity documentation quality and KPIs.
  • Lead workforce planning activities including rostering skill mix capacity management and allocation of clinical tasks in collaboration with the Executive Manager Service and Quality.
  • Support recruitment onboarding and competency-based training of clinical staff.
  • Foster a culture of clinical excellence safety accountability and collaboration.

Clinical Care Coordination & Escalation Management

  • Oversee identification assessment and management of clinical and complex care needs.
  • Support effective clinical triage and early identification of deterioration acute needs or increased risk.
  • Provide guidance on care planning goal setting risk mitigation and prioritisation of interventions.
  • Ensure evidence-based practice across aged care disability chronic disease management and mental health.
  • Act as an escalation point for complex clinical decision-making ensuring advice is safe appropriate and within professional scope.

Quality Compliance & Clinical Governance

  • Ensure clinical care complies with all relevant legislation and standards including but not limited to:
    • Strengthened Aged Care Quality Standards
    • NDIS Practice Standards
    • NSW Health clinical and community care requirements
    • National Safety and Quality Primary and Community Healthcare Standards
    • National Standards for Mental Health Services
  • Support implementation of the organisations Clinical Governance Framework.
  • Oversee high-quality clinical documentation and timely updates.
  • Participate in audits incident reviews complaint investigations and continuous improvement activities.
  • Ensure clinical incidents are documented escalated and managed in line with organisational procedures.
  • Uphold infection prevention and control principles antimicrobial stewardship and clinical risk mitigation.

Delivery of Safe Effective Person-Centered Clinical Care

  • Lead and support the team in assessing monitoring and managing high-impact clinical risks including:
    • Falls and mobility
    • Medication management
    • Cognitive impairment
    • Pressure injuries and wound care
    • Choking and swallowing
    • Mental health symptoms
    • Continence
    • Nutrition and hydration
    • Sensory impairments
  • Oversee development and review of care plans emergency plans and risk assessments.
  • Promote multidisciplinary integrated care in partnership with GPs allied health and specialist providers.
  • Ensure end-of-life palliative care and advance care planning processes are followed.
  • Support clinical decision-making regarding telehealth versus in-person service delivery.

Stakeholder Engagement & Collaboration

  • Build and maintain strong relationships with GPs primary healthcare providers and specialists.
  • Ensure timely communication of clinical recommendations reports and updates.
  • Collaborate closely with Service and Quality teams Care Coordinators and internal stakeholders.
  • Support customers carers and internal teams to understand clinical recommendations risk and care pathways.
  • Represent the organisation in clinical forums networks and sector initiatives as required.

Workforce Capability Building

  • Deliver clinical education training and coaching to the broader workforce.
  • Support access to ongoing professional development training and supervision.
  • Contribute to the development and review of clinical tools assessments protocols and resources.
  • Support policy development and review related to clinical care.
  • Promote evidence-based practice and continuous professional development.

Continuous Improvement & Innovation

  • Identify opportunities for innovation in clinical care delivery particularly in community and telehealth settings.
  • Analyze clinical data trends and outcomes to improve service delivery and customer outcomes.
  • Lead or support quality improvement initiatives focused on safety effectiveness and person-centered care.
  • Promote a culture of learning reflection and continuous improvement within the team.

  • Actively promote Work Health and Safety policies practices and procedures.
  • Maintain compliance with organisational policies procedures and quality systems.
  • Participate in performance planning mentoring and professional development activities.
  • Contribute to internal and external meetings forums and training as required.

Essential Expertise & Experience

  • Current AHPRA registration as a Registered Nurse (Division 1) (or equivalent relevant clinical qualification).
  • Extensive experience in community-based clinical care aged care disability and/or primary health.
  • Demonstrated experience in clinical leadership supervision or team management.
  • Strong knowledge of complex care and high-impact clinical risk management.
  • Comprehensive understanding of NSW health legislation and Aged Care and NDIS quality frameworks.
  • High-level assessment triage and clinical decision-making skills.
  • Excellent communication coaching and stakeholder engagement capabilities.
  • Strong digital literacy and confidence delivering care via telehealth and remote platforms.
  • Current NSW Drivers License (Class C).
