Palliative CareOverview:
Sentara Medical Group is seeking a full-time Nurse Practitioner or Physician Assistant to join its Palliative Care team at Sentara Rockingham Memorial Hospital in the vibrant college town of Harrisonburg VA.
In this role the Nurse Practitioner or Physician Assistant will provide comprehensive clinical care support and education to patients receiving palliative services working collaboratively as an integral member of the interdisciplinary healthcare team.
Position Highlights:
- Schedule: M-F 8:00a-4:30p
- No Call No Weekends
- Join an established team of 1 MD 4 APPs 1 Chaplain 1 RN and 2 MAs
- Performing palliative care consultations and follow up visits for hospitalized patients with acute palliative care needs including symptom management and complex medical decision making
- Facilitating family meetings related to current medical decision making and advance care planning
- Assisting with developing appropriate discharge plans with respect to established goals of care including close collaboration with Hospice for patients to ensure access and continuity of care.
- Actively participating as a member of an interdisciplinary team to ensure that palliative care is provided in a manner which addresses pain and symptom management; psychosocial/spiritual support; and caregiver support.
