Job Summary:
This role supports both the Medical Records and Transportation departments by ensuring accurate documentation effective coordination and continuity of patient care. Responsibilities include maintaining and securing medical records assisting with admissions and discharges coordinating patient transportation to medical appointments and serving as a key point of contact for transportation providers case managers and clinical staff. The position plays a vital role in supporting patient access to care regulatory compliance and smooth facility operations.
Key Responsibilities:
- Support daily operations of the Medical Records department in accordance with facility policies and regulations.
- Maintain assemble audit file and secure resident medical records to ensure accuracy completeness and confidentiality.
- Coordinate with nursing staff physicians and other departments to correct incomplete or missing documentation.
- Assist with admissions transfers and discharges including chart preparation and record management.
- Retrieve release and track medical records for authorized personnel and agencies in compliance with HIPAA.
- Prepare reports abstracts and documentation for physicians billing audits and regulatory agencies.
- Maintain logs registries and documentation related to medical records access and activity.
- Provide administrative support to the Medical Records Coordinator business office and other departments as needed.
- Answer inquiries and serve as a point of contact for medical records-related requests
- Coordinate and manage patient transportation needs including scheduling rides to hospitals clinics and medical appointments.
- Serve as the primary point of contact for medical transportation providers and driving professionals to ensure timely safe and compliant service.
- Collaborate with case managers social workers and clinical staff to support patient transition and discharge plans.
- Communicate transportation logistics clearly to patients families and internal stakeholders.
- Maintain accurate transportation records and documentation in accordance with organizational and regulatory standards.
- Identify and resolve transportation issues or barriers that may impact patient care or access to services.
- Support daily operations of the transportation office including workflow organization scheduling coordination and service tracking.
- Provide administrative and operational support to improve efficiency patient experience and continuity of care.
Position requirements:
- High School Diploma
- A minimum of 1 year of experience within a medical clerical position (strongly preferred)
- Ability to work and commute to Brooklyn 11228
Preferred
- Previous experience with Point Click Care
