Job Summary
SeekingRegistered Nursefor Ohels integrated Behavioral Health Clinic to provide mental health substance abuse treatment and physical health monitoring. Successful candidate will:
- Be a lead member of the Integrative Behavioral Health team enrolling and monitoring clients into the Certified Community Behavioral Health Clinic Services
- Provide primary care screening and monitoring of key health indicators and health risks(e.g. BMI blood pressure tobacco use).
- Document client care and collaborate with other members of the patient care team.
- Manage and coordinate the health care needs of clients to improve access to care and promote physical and emotional well being
- Provide high quality skilled whole-health services within a rehabilitative and therapeutic model of care
- Actively engage with clients developing positive relationships that support the client in meeting their goals
- Act as a collaborative team member leading and addressing comprehensive health promotion
- Facilitate preventative and restorative health and well-being education in individual group and community settings
- Act as a liaison between the client and external care providers for whole health needs
- Provide information and/or referrals to resources in the community for increased support and stabilization
- Offer coaching on medication management
- Document observable behaviors and symptoms
- Provide 1:1 support and intervention for people served as needed.
- Participate in meetings and communication with other providers as needed.
Current NYS RN license and a preferred 2 years of post-graduate experience. Experience in mental health a plus. Excellent salary and benefits with a flexible schedule. This part time position is located in Far Rockaway.
Salary: $ 40000
