Home Health Registered Nurse (RN)

Full Time/Field Base/Salaried/ $7500 Sign On Bonus!

What Youll Do

• Take ownership of a dedicated home health caseload

• Perform comprehensive in-home assessments addressing physical psychosocial and environmental needs

• Create implement and adjust individualized plans of care

• Complete start-of-care visits routine nursing visits and re-evaluations

• Collaborate with physicians Clinical Supervisors and care partners to ensure seamless care

• Communicate changes in patient condition and advocate for patient needs

• Maintain accurate timely documentation that supports high-quality outcomes and compliance

What Youll Bring

• Active unrestricted RN license

• At least 1 year of clinical nursing experience

• Home Health or Hospice experience preferred

• Experience with HomeCare HomeBase strongly preferred

• Associate or Bachelors Degree in Nursing preferred

• Strong assessment skills critical thinking and communication

• Willingness to travel within the assigned service area (up to 50%)

• Reliable transportation valid drivers license and current auto insurance

• Sit stand bend lift and move intermittently and be able to lift 50-100 lbs.
