Job Summary
3032441: Case Manager Assertive -Psych Admin - Rivington - Full Time Day (M-F 9am-5pm)
Assists project staff in facilitating linkage of clients from intake process to outpatient treatment
Responsibilities
new clients in supportive group treatment
as group therapist in ongoing treatment of clinic clients
with team in delivering supportive psychiatric/substance abuse treatment to assigned clients
contacts with all appropriate agencies as necessary to sustain continued engagement and progress in treatment
tracks all assigned clients in the community and facilitates coordinated treatment for clients in contact with multiple agencies
community outreach visits with clients in residences shelters etc. to insure continued viability in outpatient treatment
supportive therapeutic contacts with otherwise resistant patients
team in crisis intervention
accurate and up to date records
all subject contacts outreach visits and urine toxicology screens on a timely basis
Qualifications
- High School or equivalent BA Preferred
- Prior experience with ACT team Mobile Crisis or similar community outreach service for persons with severe mental illness or experience with community-based approaches to substance abuse treatment necessary.
- Supervisory experience preferred.
- Name: Credentialed Alcoholism and Substance Abuse Counselor (CASAC) preferred. Issuing Agency: New York State
Compensation Statement
The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $58661.00 - $67500.00 Annually. Actual salaries depend on a variety of factors including experience education and operational need. The salary range or contractual rate listed does not include bonuses/incentive differential pay or other forms of compensation or benefits.
