Full Time position

Coordinates team-based care to provide health services to individuals through effective partnerships with patients their caregivers/families community resources and physicians. Facilitates improvement in the overall quality of patient care in the out-patient care setting to ensure that optimal care coordination occurs.

  • Implements an effective internal tracking system for identified patients.

Service

  • Assists providers in the clinic with completion of documents for patient care including FMLA insurance authorizations physical forms etc)
  • Triage phone calls and provide medical advice within scope of practice and practice protocols
  • Assist with completion of medication refills per office protocols
  • Complete nurse visits for blood pressure checks
  • Facilitates coordination of care across the care continuum including: Assists as liaison with patients and their families to physicians clinical staff and other departments
  • Acting as a liaison with specialty providers; Proactively acts as patient advocate responding to and working to resolve patient concerns; Providing a link to community resources
  • Assists with clinical patient needs as appropriate
  • Performs other duties as assigned.

Finance

  • Accomplishes tasks with the minimum expenditure of time materials and money with service as the guiding principle.
  • Works productively while on the clock and offers to help others when work is completed.
  • Ensures the accuracy and completeness of clinical information for measuring and reporting physician outcomes.

Growth

  • Conducts Annual Wellness Visits to meet Medicare standards of care
  • Facilitates patient access to appropriate medical and specialty providers
  • Demonstrates the ability to positively and productively respond to change
  • Facilitates referrals and recommendations for care within the HCMH network while allowing for patient choice and best practices
  • Participates in continuous improvement offering and or implementing methods to improve care and service

QUALIFICATIONS:

The following qualifications or equivalents are the minimum requirements of this job:

Education and Formal Training: Graduate of an accredited registered nurse program. Current and valid NC license as a Registered Nurse with biannual renewal. At least 1 year experience in outpatient clinical or community health setting.

Knowledge Skills and Abilities Required: Demonstrates a high level of mental and emotional tolerance and even temperament when dealing with other team members physicians patients and their families. Must have strong critical thinking skills and exceptional ability to integrate knowledge. Excellent observation communication and decision making abilities. Demonstrates professional and effective written and verbal communication skills.

WORKING CONDITIONS:

Office environment and hospital environment with occasional exposure to hazardous conditions including blood bodily fluids chemotherapy aspects and communicable diseases. Atmosphere fluctuates from tranquil to stressful. Contact with patients families visitors employees and medical staff as well as representatives from outside the Hugh Chatham Memorial Hospital system. There is also limited exposure to hazardous cleaning solutions.
