Full Time Ongoing Contract
80 hours per fortnight - Inclusive of ADO accrual

Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute sub-acute aged care and community services.

The Stroke Coordinator at Northeast Health Wangaratta plays a vital role in ensuring stroke patients receive seamless high-quality care from the moment they present to the Emergency Department through to recovery in the community. This position is responsible for coordinating services across acute inpatient wards supporting timely transfers to subacute and rehabilitation facilities and guiding safe well-planned discharges. By working closely with multidisciplinary teams patients and families the Stroke Coordinator helps deliver consistent patient-centred care that improves outcomes and supports recovery at every stage of the stroke journey.

Wangaratta has a population of approximately 30000 and our catchment serves a community of around 100 000 throughout the northeast.
An easy 2 ½ hours drive on the Hume freeway from Melbourne and less than an hour to Albury airport with direct flights to most of the large Australian cities.
