SCHEDULED HOURS: Mon Wed & Fri 8AM-4PM Tues & Thurs 10AM-6PM

ABOUT THE PROGRAM:Transitional Support Program (TSP)

TSP is a Supported Apartment Program for Young Adults ages 18-25 and is staffed 24/7 by a multidisciplinary team consisting of an Employment Specialist Therapist Residential Counselors Administrative Assistant Residential Supervisor and Program Director. TSP provides services focused on teaching daily living skills social and recreational skills time management skills interpersonal skills conflict resolution skills coping skills and symptom management skills; with the ultimate goal of helping Young Adults transition to independent living. TSP offers these services through both group and individual modalities.

Duties & Responsibilities

Provides Direct Services and Supervision to Clients

  • Provides a safe nurturing caring atmosphere for people who reside in the programs.
  • Works within program policy and guidelines to provide program services.
  • Demonstrates an understanding of human development relevant to the age group as well as demonstrates an understanding of the actual developmental levels of the clients and intervenes to promote development and appropriately adjusts expectations by considering developmental stages.
  • Demonstrates an understanding of the importance of family/kinship connections for clients and supports those connections by welcoming parents relatives and other natural supports and facilitating planned contacts with families in the program the community and the family/relative home.
  • Demonstrates the ability to form and maintain positive nurturing relationships with clients by building trust and supporting competencies through interest and active engagement.
  • Demonstrates the ability to teach and support the learning of a broad range of skills including those related to: relationship building symptom management relapse prevention emotion regulation age-appropriate activities and competencies budgeting independent living skills. Plan and actively participate in activities to support and increase skills and competencies.
  • Demonstrates an empathic understanding of complex trauma and its effects on attachment in current relationships. Incorporates this understanding in interactions with clients and responses to trauma-based behavior.
  • Demonstrates an awareness of the physical and emotional environment by contributing to the program being a place that is emotionally and physically safe nurturing appropriately engaging and stimulating which fosters growth and resilience.
  • Supports routines through assisting and supervising in an age-appropriate way wake-up bedtime personal care routines completion of chores and laundry meals meal preparation recreational activities and homework supervision and assistance.
  • Transports adult clients when necessary.
  • Documents planned interventions behavior incidents and general observations of clients and assessment of their skills behavior and moods in an objective and professional manner.
  • Demonstrates knowledge of and role in individual treatment plans including their assessment particular needs behavioral goals and identified intervention strategies.
  • Maintains documentation in compliance with agency and program standards.
  • Participates in and leads staff meetings supervision trainings and other meetings as assigned.
  • Additional duties as required.

Provides Direct Supervision to Residential Staff

  • Directs and assists staff in crisis intervention.
  • Facilitates post crisis response meetings with program staff and program director.
  • Orients and trains new program staff.
  • Meets with assigned staff regularly for supervision and completes annual performance evaluations for assigned staff.
  • Facilitates staff meetings in the absence of the program director.
  • Coordinates staff training efforts ensuring compliance to program models.
  • Works as part of the clinical treatment team to identify clients needs and strategies to meet those needs. Oversees staff implementation of identified strategies to ensure needs are met.

Support Program Administration

  • Assists Program Director with program budget to maintain fiscal responsibility.
  • Cares for all program facilities and equipment: Household tasks and duties as assigned or as needed to promote a clean safe and comfortable environment. Maintains program vehicles and office site to agency standards.
  • Ensures program compliance with CHR health and safety standards as well as JCHAO DMHAS and OSHA standards regulations policies and procedures as applicable.
  • Provides after-hours on-call coverage including holidays and weekends.
  • May represent CHR and the program at PPTs ACRs court hearings and other meetings for client.
  • Actively participates in program development by attending all necessary committee meetings and consultation.
  • May perform other assignments and duties as needed.

QUALIFICATIONS:

Education: Bachelors in human services or related field.

Experience: At least two years working with population served in a residential setting preferred.

Licensure/Certification/Registration: Valid current drivers license.

COMPENSATION*:

$20.65/ up commensurate upon experience.

*Actual rates are determined at the time of offer and are based off of relevant experience as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note for part-time positions this range would be pro-rated based on hours worked)
