Job Summary

A growing Long-Term Care (LTC)focused organization in Monsey is seeking a Licensed Pharmacist to support partner facilities with clinical pharmacy management and medication optimization. This role plays a key part in ensuring safe effective and compliant medication use across long-term care settings while collaborating closely with healthcare providers to improve patient outcomes.

The ideal candidate has hands-on LTC pharmacy experience strong clinical judgment and the ability to work both independently and collaboratively in a fast-paced healthcare environment.

Key Responsibilities:

Manage Prior Authorizations Formulary Exceptions Step Therapy and Quality Care Dosing initiatives
Provide clinical guidance and recommendations on formulary and non-formulary medications
Collaborate with physicians nurses and facility staff to optimize medication regimens and improve patient outcomes
Conduct comprehensive chart reviews and document clinical interventions
Support and participate in quality improvement initiatives across partner facilities
Ensure compliance with LTC pharmacy regulations best practices and clinical guidelines
Assist facilities with medication-related problem-solving and clinical decision-making
Contribute to antimicrobial stewardship efforts and medication safety programs

Qualifications:

Active Pharmacist license (required)
Previous Long-Term Care (LTC) pharmacy experience (required)
Strong knowledge of therapeutic interchanges formulary management and LTC medication protocols
Excellent communication documentation and organizational skills
Ability to work independently while collaborating effectively with interdisciplinary teams
Strong attention to detail and clinical accuracy
Antimicrobial Stewardship experience is a plus
