Scope of work
Responsibility for coordinating the services provided by multiple disciplines within and outside the health system. Exercises independent judgment in the assessment diagnosis initiation of selected protocols/procedures. Provides educational support for patients/significant others health team members and the community and coordinates evaluation practices to measure outcomes of programs.
Qualifications
Required Bachelors Degree or equivalent related field
Preferred Masters Degree or equivalent related field
2 years of relevant experience related experience Required
LIC-Registered Nurse (RN) - STATEMI State of Michigan Upon Hire required or CRT-Registered Dietitian (RD) - CDR Commission on Dietetic Registration Upon Hire required or CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants Upon Hire required or LIC-Master Social Worker (MSW-Master) - STATEMI State of Michigan Upon Hire preferred or LIC-Physical Therapist - STATEMI State of Michigan Upon Hire required or CRT-Occupational Therapist Registered (OTR) - NB-COT National Board for Certification in Occupational Therapy Upon Hire required or CRT-Speech Language Pathologist - ASHA American Speech-Language-Hearing Association Upon Hire required
