General Summary: Under the supervision of the SUD ProjectManagertheSUDJusticeHospital&Communityliaisonis responsible foroutreachto members who are involved in the criminal justice system hospitals other community settingsor are currently in jail. The Liaison will coordinate with the Pima County and State courtshospital staffthe member and the treatment Teamas applicableto ensuretimelyfollow through on community re-entry appointmentadherenceandadditionalsupport during follow up liaison will coordinate andmonitorsuccessful compliance with mental and physical health RESPONSIBILITIES:Successfully coordinate with criminal justice resourceshospitals and other applicable community outpatient and peer support services to members toavoid gaps in services and to provide continuity of care (including but not limitedto:diversion educational activities med cueing environmental assessments transportation referrals ART meetings etc.). Facilitate psychoeducational groups for members involved in the criminal justice system to decrease recidivism. Complete documentation for behavioral health recordsin accordance withagency andgrantrequirements. Participate in the assessment evaluationand planning of patient SDOH assessment. Provide case management health promotions skills training and assessment when engaging with members involved in identifiedsettings. Maintain a minimum number of documented client contact hours per week. Maintain close collaboration with referral sources of identifiedsettings to continuereferrals. Provide member advocacy and education as needed to ensure delivery of required services. Perform other related duties in accordance with agency growth and changePerform other duties as assigned or necessary as it relates to the general nature of the position. QUALIFICATIONS:Minimum Education Required:Bachelors degree ina behavioralhealth relatedfield OR BHT level with two years of behavioral health :Minimum 21years of age. Able to pass a minimum 5 panel drug test & submitto regular screening. DPS Level I fingerprint clearance (must possessupon hire andmaintainthroughout employment). Maintaina clear Central Registry Background Check. CPR First Aid (mustpossess upon hire andmaintainthroughoutemployment).
