JOB DESCRIPTION:

Licensed Practical Nurse (LPN) for Amberwell Health at Home and Hospice is responsible for the care of both home health and hospice patients. The LPN will be knowledgeable in assessing home health and hospice patients and will be able to recognize complications and provide the appropriate care and implement necessary interventions under the supervision of an RN. The LPN will have well-rounded analytical knowledge organizational and problem-solving skills and knowledge of best practices in home health and hospice nursing. The LPN will have outstanding interpersonal communication and collaboration abilities.

The LPN for this position will make a difference in customer and employee experience and is committed to patient satisfaction and quality care. LPN Responsibility/duties will include but not limited to:

  • Provide direct nursing care under the supervision of a register nurse (RN in accordance with the patients individualized care plan.
  • Administer treatments as prescribed monitor patients conditions and document accurately and timely in the electronic medical record (EMR) systems.
  • Communicate effectively with interdisciplinary team members to coordinate patient care and ensure continuity.
  • Perform assessments and document patient progress flagging any concerns to the supervising RN and provider.
  • Collaborate on discharge planning with the healthcare team.
  • Answer questions and educate patients and their families on maintenance of their health and recovery.
  • Support patients and their families through the emotional and physical challenges of home health or end-of-life care.
  • Ensure compliance with infection control safety procedures and all Amberwell policies and procedures.
  • Adhere to best nursing practices standards and protocols.
  • Maintain required certifications license and participate in ongoing professional development.

Qualifications

JOB QUALIFICATIONS:

  • Experience:
    • Minimum Required Experience: 1 year home health or hospice specific care.
    • Preferred Experience: 2 years home health or hospice specific care.
  • Education:
    • Required Education: Licensed Practical Nurse
  • Minimum Required Licenses: Kansas Practical Nursing License
  • Certifications:
    • Minimum Required: BLS
