Job Summary
We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in Franklin IN.

What You Get To Do:
- Ensure that each individual receives quality individualized health care management resulting in their optimal health
- Conducting routine site visits checking vitals doing assessments counseling of medication administration and documenting progress
- Act as a health/medical resource for staff training staff in the medical arena
- Conduct audits and reviews of medications
- Ensure that the company is in compliance with state requirements and organizational policies regarding medical care and records
Schedule:
- Full-time Monday-Friday with occasional travel required; flexible hours
- Participate in an on-call rotation for triage of medical concerns for 1 week per month
Qualifications :
What Makes You A Great Fit:
- Current Licensed Practical Nurse (LPN) license in the State of Indiana required
- 1-2 years experience working with individuals with disabilities or other long-term care is preferred
- Must be skilled flexible self-motivated dependable and able to work cooperatively with staff interdisciplinary team members and outside agencies
- Strong verbal/written communication skills
- Ability to work independently and efficiently
- Organizational and computer skills
- Acceptable background check & driving record check
