GENERAL SUMMARY: In this role the Care Coordinator will be responsible for utilizing an integrated case management model that includes assessment planning implementation and monitoring. The Care Coordinator is responsible for the management of the members whole health and will provide the services and coordination that are needed/requested. The Care Coordinator will be responsible for encountering services within the Electronic Health RESPONSIBILITIES:Observe members listen to concerns and record the and/or report any areas of concerns related to the members behaviors/interactions in clinical records per agencys mission goals and management the health and safety of members the signs and symptoms of their mental illness reporting any concerns to the appropriate staff in accordance with reporting professionalism and positive role modeling for members peers and outside groups/ to accurately read record and interpret information including assessments measures and diagnostic services and coordination based on member needs/ effectively with the members support system and clinical/therapeutic as an advocate and liaison for members access to resources to support service plan in individual and team supervisions as required by licensing agency funding source or as requested by be required to transport members in personal or company appropriate providers and facilities through the continuum of care and communicate with an interdisciplinary team to develop and maintain positive working relationships with members families and collaboratively with primary care health care professionals and interdisciplinary team to offer individualized assistance with improving and maintaining quality member and update member records including assessments and treatment individual service plans that are time specific and action progress notes using the appropriate covered service billing codes and meets engagement/unit all training required by licensing agency and funding source or as requested by community resources to seek alternative and evaluate the effectiveness of the individual service for following any policies procedures and controls established by the organization the HIPAA Privacy Officer and/or the HIPAA Security Officer regarding access to protection of and the use of the an approved schedule and acceptable level of position requires the ability and mobility to perform physical de-escalation techniques as necessary and to take and pass a physical de-escalation exam. This includes the abilityto stand or sit for prolonged periods of time; to occasionally stoop bend kneel crouch reach and twist: to lift carry push and/or pull a moderate amount of other related duties as assigned or necessary as they relate to the general nature of the :Bachelors degree in a field related to behavioral health social work degree in a field related to behavioral health social work 2-years related experienceORH.S diploma and 4 -years behavioral health related experienceREGULATORY:Minimum 18 years of Level I fingerprint clearance (must possess upon hire and maintain throughout employment).CPR First Aid AED certification if required (must possess upon hire and maintain throughout employment).Current valid Arizona Drivers License and 39-month Motor Vehicle Report and proof of vehicle registration liability insurance to meet insurance requirements if current negative TB test result if required (Employer provides).Questions about this position Contact us
