The Day to Day
- Meetswith patients requiring outpatient dialysis placementorforchronic kidney disease(CKD)educationas definedbyprogram.
- Educatespatients on the progressionof disease forchronic kidney disease(CKD)transplantsdialysismodality optionsaccess optionsandinsurance.
- Demonstratesempathy for individualsbased on a thorough understanding of patients individual needsenthusiasticallypresentingStrivescare model.
- Providesa hand-offtoStrivesEnrolment Associates sharing whatwaslearned about the patients unique situation andongoing needsthrough our care management platform to enableanexcellent patient experience.
- Providesquality hand-off todepartments post-dischargeprogramifthe patient chooses to not enroll inStrivescare program.
MinimumQualifications
- 2 years combinedofrelated education experience or certification.
- Efficient and reliable transportationallowingfor the ability to commute topatienthomes or hospitals.
- Provides in-person patient care which may include standing sitting walking pushing pulling and lifting.
- Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60ms.
Preferred Qualifications
- LicensedLPN/Social Worker/Dialysis Technician.
- 1yearsexperiencein dialysissetting.
- Healthcareexperience.
- Insuranceknowledge.
- Case managementexperience.
Hourly Range: $22.12 - $25.00
