What Youll Do

The Kidney Hero Navigatoris responsible forexecutingtheKidney NavigatorProgram for patients requiring dialysisduringacutehospitalizationorathospital focuses onpatient education referralcoordinationpatient engagementandseamlesstransitiontolongitudinalkidney Kidney Hero Navigatorenhancespatientexperiencethrougheducationshareddecision-makingandadvocacyand prepares patients for ongoing management of chronic kidney position reports to Manager KidneyNavigation.

The Day to Day

  • Meetsface-to-face with patientsrequiring outpatient dialysis placement or dialysis education
  • Provides comprehensive education on kidney disease dialysis treatment modalitiesdialysisaccessoptions transplant pathways and general dietary considerations that align with patient preferences and clinical needs.
  • Collaborates with hospital case managers social workers physicians and interdisciplinary care teams toidentifyand resolve barriers totimelydialysis placement and prepare patients for dialysis initiation.
  • Acts as a patient advocate bydemonstratingempathy cultural sensitivity and a strong understanding of individual patient needs while embodyingStrivescore values
  • Provides a thorough handoff toStrivesTransitions of Care Management (TCM) team by documenting patient-specific needs preferences and circumstances within the care management platform (Health Cloud)
  • Ensures a high-quality transition of care by coordinating post-discharge handoff to the patients dialysis clinic
  • Serves as a subject matter expert in end-stage kidney disease (ESKD) including dialysis modalities transplant education patient engagement strategies and transitions of care.
  • Promotes patient satisfaction and a positive care experience throughout the referral and placement process
  • Coordinates dialysis therapy placementby referring patients toappropriate localproviders based onNephrologist order/requestgeographic proximity clinical needs and patient preference
  • Provides HIPAA-compliant renal information andrequiredmedical documentation to care teams and dialysis providerswhile adhering to all policies for secure handling of PHI.
  • Accurately documents referrals and related activitiesin accordance withprogram policies and procedures within EPIC
  • Works closely withcommunitykidney care professionals to strengthen existing partnerships and develop new provider relationships
  • Collaborateswith care teams to implement best practices for patient transitions across care settings
  • Participatesin team meetings with assigned hospital care teams and regional Strive care teams as needed
  • Performadditionalduties asrequiredto support program needs

MinimumQualifications

  • 2 years combined of related education experience or certification
  • Efficient and reliable transportation allowing for the ability to commute tohospitals
  • Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60ms

Preferred Qualifications

  • Dialysis Technician certification
  • 1yearsexperiencein dialysis setting
  • Prior healthcareexperience
  • Knowledge of insuranceprocesses and benefits
  • Case managementexperience

Hourly Range: $22.12 - $25.00
