Frederick Health Medical Group (FHMG) a highly respected 160 provider multi-specialty medical group is seeking a skilled and motivated Advanced Practice Provider (APP) - Nurse Practitioner or Physician Assistant - to join our Infectious Disease team. About the Role
As an Infectious Disease APP you will function as a clinician consultant educator and resource to physicians and the broader care team. You will help coordinate clinical services to ensure high-quality consistent and evidence-based care for patients across the healthcare setting.
This role blends direct patient care with care coordination antimicrobial stewardship principles and patient and family education - allowing you to practice at the top of your license while contributing to quality safety and population health efforts.
Key Responsibilities
- Evaluate diagnose and manage patients with acute and chronic infectious diseases using evidence-based guidelines
- Collaborate closely with infectious disease physicians pharmacists and care teams across inpatient and outpatient settings
- Coordinate care transitions follow-up planning and communication with referring providers
- Support antimicrobial stewardship and infection prevention efforts
- Educate patients and families on treatment plans medication adherence and prevention
- Document care accurately and timely in the EMR while maintaining quality safety and regulatory standards
Required Qualifications
Education & Licensure
- Nurse Practitioner: Masters degree from an accredited nursing program
- Physician Assistant: Graduation from an accredited PA program
- Active Maryland license (or ability to obtain)
- Current DEA license and Maryland Controlled Dangerous Substance (CDS) license
- Current BLS certification (American Heart Association or approved provider)
- Unrestricted license with no history of suspension or revocation
- Eligible for participation with all payers
Skills & Experience
- Prior experience in infectious disease hospital medicine or complex medical care preferred
- Strong diagnostic reasoning and attention to detail
- Comfort managing medically complex and high-acuity patients
- Excellent communication and interdisciplinary collaboration skills
- Commitment to evidence-based practice and antimicrobial stewardship
- Proficiency with electronic medical records (EMR experience preferred)
- Excellent organizational time-management and multitasking abilities
- Ability to work independently while thriving in a team-based environment
